Abstract

Purpose

“My Story” is based on a life story approach. This study aims to facilitate therapeutic alliances by providing a format for older and younger people to interact.

Design/methodology/approach

Three pairings were studied to explore the experiences of the older and younger person using “My Story”. The focus of the case studies was on how and if any therapeutic alliance emerged.

Findings

This study found that in the two of the pairings, “My Story” helped to create a bond and mutual benefit for the participants’ central to a therapeutic alliance. This led one of the pairings to develop into an intergenerational friendship and potentially help with loneliness.

Research limitations/implications

As this was an exploratory and small pilot, more cases and research are required to fully assess if “My Story” is a useful approach to develop intergenerational befriending.

Practical implications

Intergenerational befriending may be one solution that could help with loneliness and social isolation through forming a therapeutic alliance to make the befriending successful.

Social implications

Loneliness and social isolation for older people remain a problem.

Originality/value

An original pilot was undertaken to test the approach by bringing together older people identified as lonely by a voluntary sector provider and pairing these with a student volunteer. The students visited the older person over six weeks to discuss their life story and create an artefact based on the story for the older person.

Abstract

In recent years there has been an increasing focus on improving the physical health of people with serious mental illness and many policies and guidelines are now tailored to support professionals working in mental health services to meet these requirements. We found, however, that the physical health care policies produced by mental health trusts in England varied enormously. Policies were often cumbersome, vague and lacked clear guidance on what particular action should be taken, when it should be taken, and by whom. Physical health care policy documents of three mental health care trusts in the north sector of the East Midlands Strategic Health Authority (UK) were examined in detail. We found significant disparities between the policies in terms of size, readability, external references and reading cost. None could be read swiftly and all incorporated vague language into their directives. It would be beneficial for there to be more consideration given to forming local policies that are readable, succinct, and unambiguous. There is potential for considerable economy of effort with collaboration in production of these documents.

Abstract

Excess morbidity in people with enduring mental illness is well known. The promotion of healthier lifestyles and physical health monitoring has started to receive more attention in recent years. Despite this, the British Society for Disability and Oral Health (BSDH) has highlighted extensive unmet needs for inpatients with mental illness who have poor levels of oral health and hygiene compounded by restricted access to dental services. An audit cycle of oral health and hygiene was completed at Heather Close Recovery Unit (HCRU), Mansfield in 2009 and 2010, with the aims to improve the oral healthcare of the patients at HCRU and to develop the multidisciplinary team's ability to promote, monitor and enable patients to look after their dental health. A total of 59 people were helped to fill in the questionnaire during two audit runs. Improvement in access to toothbrushes increased from 68% to 86%. There is also an improvement in knowledge of basic oral hygiene practice from 55% to 61%. The ideally recommended practice of brushing teeth twice daily increased from 29% to 38% in our patients. There was a little improvement in the number of patients registered with the dentist since the last audit. We believe that prevention and early intervention are keys to addressing dental health problems in psychiatric patients. The improvement in oral/dental healthcare of patients with chronic mental illness should be seen as part of the holistic recovery package. Effective liaison with community preventive dentistry teams can play a vital role in educating mental health practitioners and patients.

Abstract

Purpose

With population projections indicating that the number of older adults in the UK (those aged 65 years and over) will increase from 9.6 million in 2005 to 12.7 million in 2021, the numbers of those growing older with a serious mental illness (SMI) is likely to rise. Currently there is a lacuna of both policy and quality research evidence on how to meet the needs of this vulnerable group of citizens. The purpose of this paper is to add to the emerging debate and discourse around current policy and practice in this field.

Design/methodology/approach

The authors have undertaken a general review of both the literature and current UK policy guidance to determine the current state of provision for people with a SMI.

Findings

Much of the relevant literature the authors identified originates from the USA, with older adults with SMI perceived as an under‐served population in need of urgent attention. Additionally, UK policy provision is patchy with very limited pockets of good practice currently available to guide clinicians and inform service users.

Research limitations/implications

Clinicians, researchers and other stakeholders must engage with service users, their family and/or friends to produce quality and meaningful research to improve outcomes for this vulnerable group of citizens. Finally, the policy, practice and service user communities need to collaborate in a spirit of “boundary spanning” working across disciplines to generate practical policies which can be operationalised by organisations and individuals.

Originality/value

This paper highlights the distinct lack of literature, evidence and policy which currently exists in the UK addressing the needs of older adults with a serious mental illness.

Abstract

Purpose

The management of violence and aggression is an important aspect of any service in the NHS and has been rightly tackled as a whole organisational approach. De-escalation is one such aspect of the organisation approach relating foremost to the safety of people and as such is a central part of relational security, personal safety and the therapeutic relationship.

Design/methodology/approach

The paper explores the evidence and policies around de-escalation.

Findings

The paper recommends that a randomised-controlled trial be designed, comparing different de-escalation techniques to establish an evidence base for this routine practice.

Research limitations/implications

The paper is limited to discussing de-escalation as an approach and the policy that directs it, and does not consider individual theories on aggression and management. It makes recommendations for policy, research and practice.

Practical implications

There is a lack of high-quality evidence around de-escalation policy and principles which staff may believe is evidence-based practice because training is often mandatory. This obviously impacts upon the patient experience and aspects of safety. The paper is valuable to practitioners working in secure environments, or with offenders that may require management of violence and aggression.

Originality/value

There are many policies and guidelines from the government and from professional bodies that seems to have tailored off since 2005 (Muralidharan and Fenton, 2006; UKCC, 2002). Forensic staff are constrained by such guidance which is further complicated by debates concerning care/coercion and forensic environments, some of which is alluded to here.

Abstract

Purpose

The paper's purpose is to reopen a debate around the potential impact of narrow conceptualisations of inclusion, or participation, of service users in current mental health policy development and implementation.

Design/methodology/approach

The approach here is a conceptual analysis of the continuity of “‘New Labour’ thinking” and its connection to Putnam on social capital and citizenship, whilst also offering counter critiques drawing on Bourdieu, Rose, and Arnstein.

Findings

The findings show the potential for disempowerment and argue for alternative service user action, either contracting on “their own rules of engagement” or specifically taking up an oppositional stance to disempowering forms of involvement. The authors also draw attention to the influence of differing English and Scottish policy drivers which appear to offer potentially different forms of engagement.

Originality/value

The paper offers a fresh analysis that particularly points to the potential value of service user groups considering alternative forms of involvement, rather than those prescribed by “Third Way” or “Big Society” thinking.

Abstract

In this chapter, we present our ongoing efforts in developing and sustaining interdisciplinary STEM undergraduate programs at North Carolina A&T State University (NCA&T) – a state-supported HBCU and National Science Foundation (NSF) Historically Black Colleges and Universities Undergraduate Program (HBCU-UP) Institutional Implementation Project grantee. Through three rounds of NSF HBCU-UP implementation grants, a concerted effort has been made in developing interdisciplinary STEM undergraduate research programs in geophysical and environmental science (in round 1), geospatial, computational, and information science (in round 2), and mathematical and computational biology (in round 3) on NCA&T campus. We first present a brief history and background information about the interdisciplinary STEM undergraduate research programs developed and sustained at NCA&T, giving rationales on how these programs had been conceived, and summarizing what have been achieved. Next we give a detailed description on the development of undergraduate research infrastructure including building research facilities through multiple and leveraged funding sources, and engaging a core of committed faculty mentors and research collaborators. We then present, as case studies, some sample interdisciplinary research projects in which STEM undergraduate students were engaged and project outcomes. Successes associated to our endeavor in developing undergraduate research programs as well as challenges and opportunities on implementing and sustaining these efforts are discussed. Finally, we discuss the impact of well-structured undergraduate research training on student success in terms of academic performance, graduation rate and continuing graduate study, and summarize many of the learnings we have gained from implementation and delivery of undergraduate research experiences at HBCUs.

Abstract

Leadership philosophy is explored through gender and other demographic characteristics in the Australian Public Service (APS), at the federal government level. Leadership philosophy is conceptualised as the leader’s attitudes, values and behaviour. Gender differences in characteristics of leaders (executives and middle managers) are examined in terms of strategic behaviour, management style, work‐related values, adoption of information technology, perceived organisational morale, family/work conflict and personal, work and family satisfaction. The gender differences are investigated using questionnaire responses from a sample of 750 respondents, of which 569 were male and 145 female. The APS findings are compared with a Cranfield study conducted in the UK’s National Health Service (NHS), where gender differences are explored in terms of management and strategic orientation. A sample of 515 chief executives, medical, clinical, HR and financial directors, chairpersons and other non‐executive directors, consists of 406 male and 108 female respondents. The APS study reveals that there are no significant gender differences in the majority of measured characteristics. Similarly in the NHS Trusts study, no significant gender differences are found in terms of management and strategic orientation. The conclusion reached is that other demographic characteristics are influential in forming leadership philosophies, namely job and organisational tenure and experience of senior management responsibilities, thus highlighting the importance of organisational demographics and their impact on leadership attitudes and practice.
